I, the undersigned, am the parent or legal guardian of the above named youth.My child has permission to attend the First Parish Milton field trip to the Unitarian Universalist Church on Arlington Street in Boston on Sunday October 27th at approximately 11am.
i authorize the adults from First Parish on this field trip to take reasonable actions to protect the safety, health and welfare of my child.I authorize any adult leader to administer first aid treatment to my child, and if necessary in their judgement, to obtain emergency medical treatment for my child. I also authorize the adult leader obtaining such medical treatment for my child to give, on my behalf, any written consentfor such medical treatment that is customarily required by medical providers, including written consent that releases the medical care provider from liability. 
I have disclosed any and all allergies, medications or other medical issues which should be known to the adult leaders or providers of emergency medical treatment.
